Victorian Numbered Acts Victorian Numbered Acts(1) The person in charge of a mental health service provider must ensure that the chief psychiatrist is notified in writing of the death of any person receiving mental health services from the mental health service provider that is a reportable death within the meaning of section 4 of the Coroners Act 2008 as soon as practicable after the person in charge becomes aware of the death.
(2) A notice under subsection (1) must specify—
(a) the name of the deceased; and
(b) the date of the death; and
(c) any other information required by the chief psychiatrist.
